Contest Rules: Hep B Stops with Me! 
OFFICIAL RULES – Hep B Moms Contest

1. To Enter: No purchase or obligation necessary. Contest runs April 22, 2010 at 8:00 am PST- to June 30, 2010 at 11:59 pm PST. To enter, you must complete an entry form and upload a photo of yourself, your child, or the two of you together. If you do not wish to submit a photo, entries may also be submitted by email to: admin@hepbmoms.org with the subject: CONTEST ENTRY.  
2. Drawing: On or about July 1, 2010, a random drawing from all eligible entries received will be conducted by Sponsor staff. Drawing  results are final. Entrants need not be present to win. All prizes are guaranteed to be awarded. Odds of winning will depend on the total number of eligible entries received. Winners will be notified by email and announced on the Hep B Moms website by July 2, 2010.
3. Prize: One (1) Grand Prize to consist of Tea Collection clothing. Value of Prize is approximately $200.00.
4. Eligibility: Contest open to all US and Canadian residents, 18 years and older as of April 30, 2010. Employees of Hep B Moms, Tea Collection, its advertising and promotions agencies, and their immediate families (spouse and parents, children and siblings and their spouses) and individuals living in the same households of such employees may submit photos /pledges/blogs, but are ineligible to win contest prizes. Void where prohibited or restricted by law. Contest subject to all applicable federal, state, and local laws and regulations. By participating, entrants agree to these official rules and accept decisions of the Sponsor as final. Void in Puerto Rico and where prohibited by law. The promotion is offered in the United States only and shall only be construed and evaluated according to United States law. Do not proceed in this site if you are not located in the United States.
5. Photo Release: By submitting my contest entry, I authorize and give my consent for Hep B Moms, Stanford University, Stanford University School of Medicine, the Asian Liver Center at Stanford University, and their employees, agents or associates to publish any publications or writings, and any photographs of me or my dependents that I submit for the purposes of this contest. If any minors (under the age of 18 years) were included in the photograph(s) I submitted, I certify that I am the legal guardian and/or parent. I expressly assume any and all risks of injury associated with, arising out of or related to the minor’s participation in this contest. I understand these photographic or electronic reproductions may be used for any purpose including but not limited to, internal training activities; quality assurance review; dissemination to health care providers; scientific or educational purposes; fundraising, advertisements, or promotional purposes; and/or publication in newspapers, magazines and other public media as may be deemed appropriate by Hep B Moms, Stanford University, Stanford University School of Medicine, or the Asian Liver Center at Stanford University.  I hereby waive any right to compensation for these uses. I and my successors and assignees hereby hold Hep B Moms, Stanford University, Stanford University School of Medicine, the Asian Liver Center at Stanford University, and their employees or agents and their successors and assignees harmless from and against any claim for injury or compensation resulting from the activities authorized by this release.
6. To Claim Prize: Winner only will be notified by Hep B Moms via email, and will be required to complete and sign an Affidavit of Eligibility and Liability Release and, where legal, a Publicity Release, which must be returned within seven (7) days after prize notification or prize will be forfeited and awarded to an alternate winner. If prize notification letter is returned undeliverable, the prize will be awarded to an alternate winner in a random drawing. Unclaimed or undeliverable prize will be forfeited and awarded to an alternate winner. Prize is not transferable or redeemable for cash. Prize is guaranteed to be awarded. Acceptance of prize constitutes permission (except where prohibited) to use winner’s name, hometown and likeness for purposes of advertising, promotion and publicity in any media without additional compensation.
7. Limitation of Liability: Sponsor is not responsible for printing or typographical errors in any related materials; stolen, fraudulent, entries or mail; or transactions that are processed or lost due to computer or electronic malfunction. Winner is solely responsible for payment of all federal, state, and local taxes on prize and for any travel or other expenses not specifically mentioned in prize description. Sponsor reserves the right to cancel or modify Contest if fraud or technical failures compromise the integrity of the Contest as determined by Sponsor in its sole discretion. This Contest offer is being made exclusively by Sponsor. Sponsor not responsible for technical, hardware, software or telephone failures of any kind, lost or unavailable network connections or fraud, incomplete, garbled or delayed computer transmission whether caused by Sponsor, user, or by any of the equipment or programming associated with or utilized in the promotion or by any technical or human error which may occur in the process of submissions which may limit a participant’s ability to participate in this promotion. Emails provided to Sponsor will remain confidential and will not be disclosed to sold to third party vendors.
8. Winner’s List: To request a Winner’s Name List, send a self addressed stamped return envelope, after June 30, 2010, to Hep B Moms. All requests must be received by July 15, 2010. Winner’s Name List will also be published on the Hep B Moms Web Site around July 15, 2010 after 12:00 pm PST.
